Гипертермия иногда развивается уже в операционной из-за укутывания ребенка и его перегревания разнообразными приспособлениями (согревающие одеяла, подогретые растворы и ин-галируемые газы). Выключение части этих приспособлений н раскутывание ребенка быстро устраняют перегревание. Повышение температуры может отражать инфекционное или иное заболевание, в этом случае необходимо установить
диагноз и назначить терапию антибиотиками и антипиретиками. Наконец, существует злокачественная гипертермия, развивающаяся обычно в послеоперационном периоде. Это заболевание, подробно описанное в гл. 4, требует немедленной диагностики и лечения.
Кровотечение после операции
Массивное кровотечение во время пребывания в ПОО может быть следствием недостаточного гемостаза во время операции или же проявлением развившейся коагулопатии. Устранение первой из этих причин требует перавода в операционную. Вторая из указанных причин уточняется при обследовании. Определяют протромбиновое время, частичное тромбопластиновое время, уровень фибриногена и количество тромбоцитов. Эти показатели позволят провести дифференциальный диагноз между дилюциониой тромбоци-топенией, дефицитом факторов свертывания и синдромом диссеминированного внутрнсосуди-стого свертывания (ДВС-синдром) (см. гл. 6). Терапия (трансфузии плазмы, тромбоцитов или криопреципитата) при каждом из этих состояний различная.
